Empowering Women: Investing in Contraceptive Research and Innovation for a Healthier Future

The availability of contraceptives has revolutionized women's reproductive health, empowering them to make choices about their bodies and plan their families. However, there is a pressing need for new and improved contraceptive methods, particularly in low- and middle-income countries (LMICs), where unmet needs for contraception and dissatisfaction with existing methods remain high. Unfortunately, the sector faces significant challenges due to limited funding for research and development (R&D), especially in Africa and other LMICs. This article sheds light on the current state of contraceptive R&D, highlights encouraging trends, and calls for collective action to bridge the funding gap and accelerate innovation.

The Funding Gap

The landscape of contraceptive R&D is hindered by several factors. A challenging regulatory environment, focused on healthy individuals, creates barriers for product development. Moreover, the potential for litigation and perceived low returns dissuades many investors from supporting contraceptive R&D. Shockingly, funding for contraceptive R&D has stagnated since 2018 and even declined in recent years. In comparison, funding for other sexual and reproductive health issues, such as LMIC-relevant HIV/AIDS R&D, receives significantly higher investments. Furthermore, the disparity is disheartening when considering that the expenditure for cosmetics and personal care in 2021 exceeded the funding for contraceptive R&D.

Encouraging Trends

Despite the funding challenges, there are promising developments in contraceptive innovation. A shift towards human-centered and user-centric design is evident, as reflected by decreased investments in hormonal and non-user-controlled methods. This shift indicates a growing emphasis on meeting the evolving needs and preferences of contraceptive users. Similarly, there is a move towards gender-equitable healthcare, as the proportion of funding allocated to female-only contraceptives has decreased. Recognizing the importance of involving men in family planning, there is an acknowledgment that a broader range of contraceptive options is necessary to address unmet needs comprehensively.

Early-stage Development

While progress is being made, next-generation contraceptives are still in the early stages of development. Only a fraction of the funding (38%) over the past four years has been allocated to early-stage research, whereas clinical development and post-registration studies receive a meager 23%. Notably, non-hormonal, user-controlled, and male-only contraceptive R&D receive a higher proportion of funding for early-stage research. This implies that it will take time before these innovative contraceptive methods reach those who need them the most.

Dominance of Funding and Vulnerabilities

Small to medium-sized pharmaceutical and biotech companies receive minimal funding, while there is virtually no support from the public sector in LMICs or public sector multilaterals. Such concentration of funding poses a serious vulnerability, as the interests, priorities, and budgets of a limited number of parties dictate the course of contraceptive R&D. To achieve global sexual and reproductive health and contraception commitments and targets by 2030, it is imperative to diversify funding sources and engage new funders from all sectors.

To ensure a healthier future for women and communities worldwide, concerted action is needed to address the funding gap and promote innovation in contraceptive R&D. The following steps are essential:

  1. Increased Funding: Existing funders must continue their support and commit to greater investments in contraceptive R&D. New funders from diverse sectors, including the public sector in LMICs and public sector multilaterals, should step forward to contribute.
  2. Collaboration and Partnerships: Collaboration among stakeholders, including governments, research institutions, non-profit organizations, and private companies, is crucial to drive progress in contraceptive R&D. Partnerships should be fostered to share resources, expertise, and knowledge, promoting a collective and comprehensive approach to innovation. Governments in LMICs should prioritize reproductive health and allocate funds towards R&D efforts, fostering a supportive environment for research and innovation.
  3. Inclusive Research Agenda: Contraceptive R&D must prioritize a user-centered approach, involving the perspectives and experiences of women, men, and marginalized communities. Research should focus on understanding their unique needs, preferences, and cultural contexts. Inclusivity in clinical trials and research studies is vital to ensure that contraceptive methods are effective and safe for diverse populations.
  4. Regulatory Reforms: Governments and regulatory bodies should streamline the approval processes for contraceptive research and development. This includes establishing clear guidelines and frameworks that consider the specificities of contraceptive products and prioritize public health over potential litigation risks. Encouraging an enabling regulatory environment will attract more investment and expedite the availability of new and improved contraceptive methods.
  5. Capacity Building: Building research capacity within LMICs is essential to foster local innovation and ensure sustainable progress in contraceptive R&D. This includes supporting training programs, providing resources, and strengthening research institutions and infrastructure. By empowering researchers and scientists in LMICs, we can nurture a self-sufficient environment that addresses the unique challenges and needs of their communities.
  6. Public Awareness and Education: Efforts should be made to increase public awareness about the importance of contraception, debunk myths and misconceptions, and promote the use of modern contraceptive methods. Comprehensive sexual education programs should be implemented to empower individuals with accurate information and promote informed decision-making.
  7. Advocacy for Policy Change: Civil society organizations, activists, and advocates should champion the cause of contraceptive research and innovation. By engaging in policy advocacy, they can raise awareness, mobilize resources, and influence policymakers to prioritize reproductive health and commit to adequate funding for contraceptive R&D.
